Community Coalition, For Health
Community Coalition for Health (C2H) is a community health coalition in Texas that connects families in Austin's Eastern Crescent to care, knowledge, and leadership. We offer trusted community health resources in Austin so no one has to figure out the health system alone.
Since 2014, our team has worked side by side with neighbors, schools, churches, and local partners. We help people find coverage, learn about chronic conditions like diabetes, and grow into leaders who can improve health on their own blocks.

Dr. Charles A. Moody Jr. and Mia Greer started C2H in 2014 with one clear goal. They wanted to bring health awareness and real support to communities in east Austin that were too often left out.
Their early work focused on diabetes. Many families were living with it, yet few had simple, trusted guidance on how to manage it. C2H built a diabetes education model that has been approved by the American Diabetes Association. That same hands-on, research-based approach now shapes every program we offer.
Today we are a health equity nonprofit in Texas that looks at the whole person. Health is shaped by more than a doctor visit. It is shaped by housing, income, school, safe streets, internet access, and even the summer heat. So we work on all of it, together with the people who live it every day.
We believe people are the experts on their own communities. Our job is to bring the tools, the facts, and the connections, then stand with families as they lead the way.
Our mission is to empower, inform, and inspire residents to take charge of their health through accessible education, clinics, and community wellness programs in Austin. Our vision is a city where every family has the knowledge and tools to live well and thrive, no matter their zip code.

The word "coalition" is in our name for a reason. Lasting change happens when community members, researchers, health systems, and faith leaders work as one team.
Our partners help us bring research, data, and resources straight into the neighborhoods that need them most. This underserved community health support reaches people through churches, schools, and local events they already trust.
Climate and health in action. Through Austin CARES (Climate Adaptation Resilient Communities), supported by NASA Earth Observations, C2H helps combine satellite data, public health data, and community knowledge. The goal is to protect at-risk neighbors from extreme heat and guide smarter local policy.
Our team brings decades of experience in nursing, public service, research, and community outreach. Most of all, we bring a lifelong commitment to the families we serve.

Dr. Moody leads C2H's research-based work on health gaps, diabetes, and climate resilience. He holds a Doctor of Strategic Leadership and has served Central Texas as a pastor, police chaplain, and educator for over three decades.

We serve individuals and families across Austin and Travis County, with a focus on the Eastern Crescent. This includes low-income families, people facing housing changes, students, seniors, and anyone who needs help finding care.
Our navigation support, education programs, and community events are offered at no cost to participants. Some programs, like the Community Health Leadership Fellowship, even pay participants for their time.
